Award medal with green and white vertical striped ribbon with brooch pin. Medal is brass colored in the shape of a cross. The obverse has the raised image of a horse head, cat, dog, and bird on the cross arms going clockwise. Around the center circle is raised lettering "THE HUMANE SOCIETY OF NEW YORK." In the very center is raised lettering "FOUNDED / OCT. 1904" surrounded by a laurel wreath. The reverse side is undecorated and has engraved lettering "MRS. VELMA B. JOHNSTON / "WILD HORSE ANNIE" / FOR OUTSTANDING HUMANE EFFORTS / 1960." Maker's mark stamped on back at bottom "M.S. BOWMAN / N.Y.C."
